Cremation Forms
Please print forms on legal size paper. All forms must be appropriately signed and stamped.

Form A - to be completed by Next of Kin and Justice of the Peace.

Form B - to be completed by the Senior Superintendent of Police.

|
Form C - to be completed by the Medical Practitioner who has attended to the deceased during his/her last illness and who can certify as to the cause of death.
| ||
|
Form D - to be completed by the Senior Medical Officer/Government Officer who is not a relative of the deceased or a relative/partner of the medical practitioner who gives the certificate in Form C.

|
Form E - to be completed by the Pathologist if a post-mortem examination (autopsy) was done.
